Family member benefits eligibility verification

So that you are not surprised if you receive a UC communication regarding family member benefits eligibility verification, we are sharing this important information. To ensure that UC’s benefits resources are used wisely, UC is coordinating with UnifyHR to confirm family member benefit eligibility for some enrolled family members. 

Who will receive the eligibility verification request and when?

  • If you enrolled a new family member or members in your UC coverage during Open Enrollment, you will receive a request to reverify between now and November 2025.
  • If you completed verification more than 3 years ago for the following dependents: spouse, domestic partner, stepchild, grandchild/step-grandchild, legal ward or overaged disabled child, you may receive a request at any time throughout the 2025 plan year.

Action required

  • Employees must respond by the deadline specified in the letter to maintain their family members’ coverage.
  • Please ensure your contact information is correct in UCPath and keep an eye out for a packet from UnifyHR.
  • If you have any questions, please reach out to UnifyHR after you receive your packet. Customer service contact details will be included.
